hi.......everyone........this is my first bog post n do u know wt made to write this post?????? a very curious case that came across me during my dental practice.....its a must for a dentist to knw a patient's medical history in detail before proceeding to a dental treatment. This girl aged 23 came to clinic regarding a dental problem. but now my concern is not just her dental problem but this unidentified problem dat she's living with. her last memory of the symptom was when she was 6-7 yrs old.
she said she cant bear pain n gets fits when she's in pain. sometimes its assosiated vid urination too.
apart from pain there's nthng dats assosiated vid her fits. her parents told me dat she tends to clench her teeth during her fits. n never gets these fits when she's normal or pain free.
n its not dat everytym she's in pain n she gets fits. it happens sumtyms n gap can be months or years, mostly when she's undergoing some tensions in lyf. her EEG was done when she was around 10 yrs n it was normal n d concernd doc said she wud get normal as she'll grow old. after dat she never visited ne doc for the disease.
but now her concern has increased for d disease cuz of her marriage.
can neone help me regarding this case n suggest me what she should do.
n r there ne precautions dat i shoud take during my dental procedures for her.

Exact location of pain ? Neuralgia ? The vagoglossopharyngeal neuralgia is associated with syncope. A trial of carbamazepine may be useful.

I hope he will come to a final diagnosis soon. Mostly, there wont be any changes in the EEG. Once you post the followup here, i can give my suggestions.

thanx doc..... i hv referred her to a neurosurgeon. n have successfully completed her dental treatment for today.

her ct scan of brain should be done

and more detailed history of past retaken again from her family to ascertain epilepsy,which seems to me a good probability until and unless proved otherwise

thank u so much doc for ur precious comment iv passed it to my patient whose vid me ryt nw n u are ryt her problem doubles during menses........ she has a new question dat if she should start vid d treatment or its not required n if yes which wud be better homeopathy or allopathy......what do u suggest for her???? n its a concern for me dentistry is pain free but patient may experience sum amount f pain n in this case i do have to take precaution........

Many parents complain that their child is having epilepsy. When we take the history in detail, we confirm that it is febrile convulsions, infantile spasms, or something like that. Similarly, here the convulsion is associated only with pain. It may not be epilepsy, but shows the hypersensitivity of this girl to pain. The problems may get doubled during periods. For convulsions with pains, we homeopaths give drugs like Belladonna 200, arnica 200, Cicuta 200 (as per the indications). Belladonna is the drug indicated for convulsions during every attack of pain. The same medicine in the form of Atropine is given in modern medicine before any surgical procedures. You can take the opinion of a neurologist before doing the dental procedure. We beginners should always take such precautions!
