first of all it is flail not flial...
when 4 or more than 4 ribs are fractured anteriorly and posteriorly both side then this segment moves inversly with respiration i.e. inward with expiration and outward with inspiration.this segment is flail segment and respiration is parodoxical respiration.

the situation here is not very encouraging but there is no harm in trying...u will have to give IELTS and PLAB and try for training job...u may have to do jobs in medicine initially and then try and get in dermatology

oh sorry as here 1st year of residency is called 'internship'. if u r talking abut externship i.e work without pay thats in some NYS programs . u do hands on experience on pts like doing h&p ,phys exam help the doctors but only get a letter of reference in the end. they r not that easy to get either but u never know if u r lucky. without that nobody offers paid work .

i am not sure where u graduated from but i believe u have to take mrcp there , all i know its tough for img's there. i myself am in usa so can't say much about uk. feel free to ask about usa.
